At Alves & Martinez, we make every attempt to ensure that the process is as clear and easy as possible. We understand paperwork can be tedious and frustrating. As a licensed medical practice, we are required by law to provide patients with certain documents.

To minimize any delay initiating your first visit with us, please complete the necessary documents provided below. At the time of your evaluation, bring your paperwork with you. Remember to arrive at least 15-20 minutes early, as you may also need to complete additional medical questionnaires specific to your condition.

We’ve partnered with KEET Health to measure our effectiveness and outcomes. This process requires additional data input (in paper form or via tablet) at your first visit and during re-evaluations to measure the effectiveness of our interventions.
